When the three-step process (Redirection, Mediation, Restraint) is initiated, a formal Behavioral Intervention Report is created. This report includes:
The specific behaviors that led to the intervention.
The duration of the restraint and the methods used (e.g., Basket Hold or Pinel system).
The names of the staff involved (Nurse, Psychiatrist, Security, etc.).
Medical observations, including any medication administered by injection.
Records of restraints are considered Protected Health Information (PHI). Access is strictly limited to:
The Clinical Team: For the purpose of ongoing treatment and safety planning.
The Hospital: Information will be shared with the receiving hospital during transport to ensure continuity of care.
Legal Guardians: Parents or guardians have the right to review the incident report and discuss it with the attending Psychiatrist.
We do not share details of your child’s behavioral incidents with unauthorized third parties. Information is only disclosed outside of our facility when:
Required by law or state regulatory agencies for safety audits.
Necessary for emergency medical treatment at a hospital.
Required by a court order or protective services.
During an intervention, we prioritize your child’s privacy. In the event a parent is asked to leave the room to de-escalate the situation, staff will ensure the area remains clear of other patients and non-essential personnel to maintain the child’s dignity.
All incident reports are stored securely within your child’s permanent medical file. These records are kept for the duration required by state law and are destroyed or archived according to medical record retention schedules.
